Minors must be accompanied by their parent/guardian and both parties must produce valid I.D.   The parent will fill out and sign, along with the child, a parental release form, and copies of the id's will be made.   The parent must also approve of the design, size and placement of the child's tattoo.   This must be done every time the child wants a tattoo.   No exceptions will be made.
